there’s a rare mental illness called Body Integrity Dysphoria which makes people want to amputate a limb. They have less phantom pain than ordinary amputees, supposedly.

Define unnecessary.
Issue is, people with Body Integrity Dysphoria might feel extreme discomfort from having a cartain limb (or more), those who gotten said limb amputated got their life improved. They still sometimes suffer from phantom limb pain, but often was an acceptable alternative to having said limb.
Even worse is, eugenicists often think even medically necessary amputations are unjustified, since “a disabled man is just as good as a dead man”.
