The shifting pronouns midway through threw me off for a while because I couldn’t track how many characters were active in the scene. Basically, though, the lord is transfem. The farmer addresses her as female (and maybe compliments her chest, not sure). She thanks him, because she was boymoding (attempting to present as male because that’s what’s expected of her), but the farmer recognized her as female anyway.

Estrogen in pill form was originally derived (and in some cases still is!) from pregnant horse piss.
The trans noblewoman is collecting the piss levy to make her estrogen.

I think (though I have low confidence in it here) that he is complimenting her rippling muscles, which isn’t necessarily what the peasant sees, but what the peasants thinks is appropriate for someone in armor.
Fuck If I know what ‘her “male faileth”’ is supposed to mean, though.

It’s less that I’m confused about what’s happening on a surface level (though some of that is indeed a bit unclear with all that fake-old timey speak), and more that I’m confused what the OP actually wants to express with this story. Like, is this pro trans or anti-trans?
