Not all people with a colostomy bag have their anus sewn shut, but everyone with their anus sewn shut have a colostomy bag.

Some do, some don’t. Depends whether they/you think you’re getting hooked back up eventually.
I suppose it might be possible to have an injury that results in not having an anus, but it’s not an inherent part of having a colostomy bag.
