Kaiser isn’t a typical insurance company though. They’re an HMO. Which means they tell you what doctor you can go to. Think you need to see a specialist? Too bad, the general practitioner doesn’t think it’s necessary.
So they deny a lot of claims by preventing you from going to the right doctor in the first place.

Kaiser Permanente is the largest HMO in California, where state healthcare regulations are stricter than other states, and they have an individual mandate that comes with actual penalties. They are one of the least evil health insurance companies, but they are still health insurance, an inherently evil concept that leeches value from the healthcare industry at the cost of human lives.

Its not just CA regs, Kaiser is an enclosed ecosystem. The doctors, facilities, insurance, is all the same company.

Which, itself, causes a lot of problems.
