It’s hard to give a blanket answer because it varies from league to league. The NWSL does provide health and life insurance to all players under the current CBA and the league does cover injuries that take place on the pitch. Similarly, if a player takes an extended absence for maternity, they continue to draw their salary. Other leagues have other policies, generally determined by what their players’ union has been able to negotiate.
Do the major leagues have insurance for their players for life? If they get hurt on the field and it's something that will never go away? Or do they just cut the player and not worry about it?

That’s what the Players Unions are always trying to get.

I watch NFL and MLB, so those are the leagues I can speak on. If you’re actively playing, you’re insured. Doesn’t really matter, though, since all the teams have their own medical staff, so there’s no expense to insure against.
After you retire, depends on the league, depends on the union. MLB players union is crazy strong. If you play in one MLB game, you get medical insurance for life. You also get a pension of you play some small number of games (50ish?).
NFL players union is much weaker, relatively. If you play in 3 games per year, you get insurance for the following the years. After that, you’re on your own.

How does one play in an MLB game with no skills of any kind?
