Depending on his weight that could be quite a high dose of ivermectin (the normal dose is 0.2mg/kg) but that seems like an extreme reaction.
I see three possibilities:
- He’s very light weight and sensitive to the drug
- He’s having an allergic reaction
- He actually did have a parasite infection and he’s having a Mazzotti reaction.

Mebendazole sometimes causes diarrhea, abdominal pain, and elevated liver enzymes. In rare cases, it has been associated with a dangerously low white blood cell count, low platelet count, and hair loss,[14][15] with a risk of agranulocytosis in rare cases.
