Depending on his weight that could be quite a high dose of ivermectin (the normal dose is 0.2mg/kg) but that seems like an extreme reaction.

I see three possibilities:

He’s very light weight and sensitive to the drug
He’s having an allergic reaction
He actually did have a parasite infection and he’s having a Mazzotti reaction.
