AI could make things a lot easier though. Scan constantly for words that look like a particular thing and change those words on the fly, for example, can allow for the type of censorship you're asking about.

Verbal stuff might be harder unless you put in a delay though (or you're dealing with pre-loaded media that can be censored as it buffers) because you don't know what a word is going to be until it's uttered.
