According to Vaati, from interviews given to Japanese magazines, the entrance to the DLC is Miquella’s arm coming from the cocoon at the back of Mogwyn palace. So youll likely have to at least get past Mogh, Lord of Blood, to enter.

Rumor is you need to interact with a certain cocoon in a certain boss room. Wink wink
