In Baldur’s Gate 3 at least (almost) all underwear materialise a bra when you pick body types two or four, so can go fully nude but you can’t really wear just pants.

Ah, yeah. Well you can wear pants and no underwear at least. Plus it’s easily moddable, but point taken.
