What can the nationalised operator do to reduce the number of cancellations? Is there a way to tell if they’re at fault, or if the cancellations are down to the Network Rail side of things?

There has been. The DfT employs thousands of lawyers whose job it is to assign blame ro every single delay on the rail network, for the purpose of determining whether it’s NR or the TOC who pay the costs incurred.
That’s likely getting scaled down now that GBR will be answerable for a a majority of passenger service in England and also the infrastructure.
